Unless otherwise stated, the forms listed below can be completed by following these steps:

2. You may also navigate to Files & Forms by clicking your name at the top right of the screen, or clicking the link in the banner notifying you of Forms to Review
3. Complete the form and click "Submit"

Medical History and Allergy Information

Please share if your child has any allergies, including food allergies, or medical history that the school should know about. For any allergies and/or medical needs that require medication to be distributed while at Greenhills, the Medication Authorization form must also be completed. If your child has an Allergy or Medical Action Plan, please email those to healthservices@greenhillsschool.org.

Dietary Preferences
Please use the form below to share any special dietary preferences your student may have, such as kosher, halal, vegetarian, or vegan. This information will also be shared with our lunch service partner, Plum Market. For food allergies, please complete the Medical History and Allergy Information form in GryphOn.

Medication Authorization

Prescription Medication
Michigan law requires a physician’s written order, along with the parent/guardian signature of authorization, for administration of all prescribed medications, including EpiPens and inhalers. A new form must be provided each year.

1. Download the Medication Authorization form
2. Fill in your child’s information (a physician signature is required for prescription medication)
3. Visit the Files & Forms page in GryphOn
   You may also navigate to Files & Forms by clicking your name at the top right of the screen, or clicking the link in the banner notifying you of Forms to Review
4. Upload a scan/photo of the completed and signed form(s) and click "Submit"

Non-Prescription Medication
For non-prescription drugs used on an occasional basis, such as Benadryl or ibuprofen, only a parent/guardian signature of authorization is required. If you would like to send over-the-counter medications that Greenhills doesn't supply (Claritin, Dayquil, etc.) for your child please email healthservices@greenhillsschool.org. The list of medications supplied by Greenhills is on the form in GryphOn. 

New Families: Submit Immunization Records

Before a child can enroll in a new school district in Michigan, parents or guardians are required to produce documentation that confirms their child has either received the school-required immunizations or has received at least one dose of each of the required immunizations and is awaiting receipt of subsequent doses to be administered at appropriate intervals. Please see the Student and Family Handbook for more information about immunization requirements.

Concussion Information Acknowledgement

Michigan's Sports Concussion Law requires schools to provide educational materials on the signs, symptoms, and consequences of concussions to each student athlete and their parents/guardians and obtain a signed statement acknowledging reciept of the information. 

Emergency Contacts

Please provide current contact information for trusted adults—such as relatives, friends, or neighbors—who routinely assume temporary care of your student if a parent or guardian cannot be reached in an emergency. Keeping this information up to date helps the school respond promptly in the event of an illness or injury.

The bus routes below, with the exception of TheRide, are determined based on input from families who utilize those services. If you’re considering any of these options, please complete the applicable forms. 

Greenhills Parrent Organization (GPO)

All parents are automatically members of the Greenhills Parent Organization (GPO). This year's GPO grade reps are linked below, and they will contact you regarding opportunities to connect, engage, and volunteer throughout the year.
